How the Process Works

Five clear steps from your first call to your last contact with us.

    Fill out our brief contact form or call our office. Someone from Mind Matters will personally call you within 1 business day.

    Brief Intake Consultation

    We'll discuss your concerns, answer your questions, and determine the right service for your child or for you.

    Schedule Your Appointment

    Appointments are typically available sooner than the months-to-even years wait time that can be encountered in other settings.

    Evaluation or Session

    You'll work directly with Dr. Zaccariello and/or Mrs. Bailey or Kuehn-Garteski.  Every patient experience receives board-certified clinical review.

    Pediatric Neuropsychological Report and Feedback Session

    Receive a thorough, actionable report and a personal feedback session. We'll walk through findings, answer every question, and connect you to next steps and community resources.

    ADHD/Executive Function Coaching

    Individual 30 to 60 minute ADHD/Executive Function coaching sessions with Mrs. Bailey.

  3. 5c

    Adult Psychotherapy

    Individual 60-90 minute psychotherapy sessions with Dr. Zaccariello to address mental health concerns.

Investment & Pricing

We believe in transparency. Here's a straightforward look at our fees — no surprises, no hidden costs.

Psychological & Neuropsychological Evaluations

Each encounter is a comprehensive, multi-session process — not a one-time appointment.  Evaluation fees cover clinical interviewing, record review, neuropsychological test administration and scoring, report writing, and an individualized feedback session.

ADHD / Learning Disability Evaluation

$4,000

  • Clinical intake interview
  • Record review
  • Standardized cognitive and achievement testing
  • Parent and teacher questionnaire review
  • Written report with background history, relevant medical history, evaluation findings, and recommendations
  • 60-minute feedback session

Complex Medical / Neurological Evaluation

$4,500

  • All components of the ADHD/LD evaluation
  • Expanded neuropsychological battery
  • Coordination with referring providers, therapists, and/or school personnel as needed (additional charges possible)
  • 60-minute feedback session

Autism Spectrum Evaluation

$4,750

  • ADOS-2 administration
  • Targeted developmental and social history interview
  • All components of the ADHD/LD evaluation
  • 60-minute feedback session

Psychotherapy & Coaching Services

Counseling

Intake Package (2 sessions)

$600

Your intake includes two 60-minute appointments. The first session covers your full history; the second focuses on goals and builds your care plan.

Individual Sessions

$250 / hour

Ongoing therapy sessions scheduled at a cadence that works for you.
$300 for 90-minute sessions

ADHD/Executive Function Coaching

Sessions

$100 / hour

Coaching is skills-focused and practical. We work on the real-world challenges of ADHD and Executive Function weaknesses (e.g. planning, prioritizing, time management). 

Coaching is not psychotherapy and does not involve diagnosis or treatment of mental health or neurological conditions.  Additionally, ADHD/Executive Function Coaching is not typically reimbursed by medical insurance.

School Meeting or Observation; Treatment Plan Meeting

$300 / hour (including travel time)

At family request, Dr. Zaccariello may conduct a school observation, attend an IEP or 504 Plan meeting, or a treatment plan meeting.

Insurance & Payment

As a private-pay practice, Mind Matters does not bill or accept payment directly from insurance. This allows our clinicians to spend the time necessary for a truly comprehensive evaluation, select the most appropriate and up-to-date tests for each individual, and base every decision on clinical need rather than insurance restrictions. We provide a superbill after your evaluation so you can seek out-of-network reimbursement directly from your insurer, if your plan allows.

We recommend contacting your insurance provider before beginning services to ask about your out-of-network mental health and neuropsychological testing benefits.

Payment is accepted via debit or credit card, Health Savings Account (HSA), Flexible Spending Account (FSA), cash or check.  A 3% processing fee will be charged for credit card usage.

What is a superbill?

A superbill is an itemized receipt that includes diagnosis codes, procedure codes, and provider credentials. You submit it directly to your insurance company, and if you have out-of-network benefits, they may reimburse a portion of the cost.

Questions and information for your insurance provider?

Here are some questions you can inquire with your insurance provider: What are my out-of-network benefits? What is my out-of-network deductible? Are these some of the possible Current Procedural Terminology (CPT) codes that will be used for a neuropsychological evaluation: 96116, 96132, 96133, 96138, 96139. Are these possible CPT codes for individual psychotherapy: 90791, 90837.

Do you offer payment plans for testing?

We understand evaluations represent a significant investment. Please reach out to discuss options — we're happy to talk through what works for your situation.

How do I know which evaluation I need?

We'll help you figure that out during your initial consultation. You don't need to decide before you call.

Fees, Insurance & What to Expect

Mind Matters is a private-pay practice. We do not bill insurance directly. We provide detailed superbills that you can submit to your insurance company for potential out-of-network reimbursement.

Do you accept insurance?

We are a private-pay practice, meaning payment is due at the time of service. We will provide a superbill — an itemized receipt with diagnostic and procedure codes — that you can submit to your insurance for potential out-of-network reimbursement. Many families with PPO plans receive partial reimbursement.

What is the cost of a neuropsychological evaluation?

Fees vary depending on the scope of the evaluation. Please contact us for current fee information. We are committed to transparency and will discuss all costs during your intake call.

Do you offer payment plans?

Please speak with our team — we want to ensure that financial concerns do not prevent families from accessing the care they need.

Do you work with Mayo Clinic employees?

Yes. As a private, community-based practice, we provide a confidential option for Mayo employees and their families who prefer care outside the health system.

How long does a pediatric evaluation take?

Most evaluations require one to two full days of testing, followed by a feedback session. We will provide a clear timeline during your intake consultation.

Do you see patients from across SE Minnesota?

Our office is in NW Rochester. We serve families from Byron, Stewartville, Kasson, Austin, Owatonna, Winona, and communities across Southeastern Minnesota and Southwestern Wisconsin.
